Dedicated to home health and community-based health care, VNHSC cares for families living in 25 communities throughout eastern and north central Connecticut.

Palliative Care Services

Our palliative care program provides symptom relief and pain management to help stabilize and improve quality of life among patients experiencing physical and emotional discomfort related to the progression of a disease or illness. Our palliative care program is designed for individuals who are currently living at home or in an assisted living facility, and wish to remain out of the hospital setting.

 

Hospice Care Services

Visiting Nurse & Health Services of Connecticut (VNHSC) has been providing hospice care services to the community for over 30 years. Hospice Care uses an interdisciplinary team to provide intensive comfort measures to enhance the quality of life during the final stages of a terminal illness to patients and their families. Hospice is provided to patients who live at home, live in an assisted living community or reside at a skilled nursing facility.

 

Wound, Ostomy and Continence Services

Visiting Nurse & Health Services of Connecticut (VNHSC) offers Wound, Ostomy and Continence (WOCN) services to those who need it. Our care team will provide a thorough assesment of the wound, educate clients and families on the best way to take care and manage the wound including vitamins and nutritional supplements and faciliate post-discharge supply coordination and appropriate support groups.
