Sarah Yiznitsky is a Board-Certified Family Nurse Practitioner specializing in Palliative and Hospice Care, as well as Primary Care. She began her medical career in 2011 after earning her Bachelor of Science in Nursing from Saint Anselm College. Since then, she has gained extensive experience in hospice care, both in inpatient and community settings, as well as acute care in hospital environments.
In 2017, Sarah earned her Master of Science in Nursing from the University of Connecticut, followed by a Certificate in Pain Management in 2018. She further advanced her expertise by completing her Doctor of Nursing Practice (DNP) at the University of Connecticut in 2022.
Sarah is dedicated to collaborating with patients and staff to manage chronic conditions and facilitate meaningful discussions about Goals of Care.
Sarah provides in-home primary care services for patients of ECHN Medical Group’s Primary Care Practices. To receive in-home primary care services patients must be homebound and referred by their ECHN primary care physician.
For Palliative Care consultations, any physician or patient may call the Ellington Primary Care office to schedule an in-home consultation.
