Our Commitment to Provide Safe, Quality Care

Our Commitment to Advancing Health Equity

We recognize that equity must be a strategic priority to ensure exceptional care for all individuals. Guided by the five CMS domains for health equity—Equity as a Strategic Priority, Data Collection, Data Analysis, Quality Improvement, and Leadership Engagement—we are embedding health equity into every aspect of our operations. By addressing social drivers of health (SDOH), collecting and utilizing REaL (Race, Ethnicity, and Language) data, and providing cultural sensitivity training to our staff, we aim to eliminate disparities and create a healthcare system that delivers equitable outcomes. Our referral systems also connect patients to essential social services, ensuring continuity of care and a holistic approach to health and well-being.

Targeted Initiatives to Reduce Health Disparities

As part of our commitment to health equity, we are leading three impactful initiatives to close gaps in care and improve outcomes.

  • For diabetes care, we are addressing disparities among Black Americans, specifically targeting the prevalence of diabetes among this population. There is a significant emphasis on providing interventions to address diabetes' associated complications, such as lower-leg amputations and mortality. Additionally, there are a variety of strategies implemented to address preventive management with comprehensive treatment programs. 
  • In women’s health, we are providing no-cost screenings and follow-up care for underinsured populations across Connecticut, ensuring that essential preventive services are accessible to all women.
  • For behavioral health, we are eliminating transportation barriers by offering services that enable patients to access therapy sessions and treatments. These initiatives reflect our dedication to reducing disparities, improving access to care, and fostering a healthcare environment where equity and quality go hand in hand.
